All fields marked with an asterisk are required and must be completed.  Failure to complete all required fields will delay your proposal.

BE SURE TO FOLLOW THE STEPS, IN ORDER.

STEP 1.  Select your department by clicking +Add Item. (see help text)

  • Department*
Click on +Add Item. A drop-down menu will appear; scroll to select your department. Once you select your department, in the right pane the appropriate individuals/committees will populate based on your department/school
  • STEP 2.  Complete the next four fields.

    • Select Program if you are making changes/edits within your entire program.

    • Select Shared Core if you are only making changes to a programmatic core that is shared among several academic programs, or shared among other programs within your own department.

    • Select the level of change (Level III requires SUNY/SED approval).  Please refer to the Curriculum Change Guide to determine your level of change, or call the Academic Affairs Office.

    • Select the action being requested.

  • Program Type*
  • * Select One
  • * Level of Change
Please refer to the Curriculum Change Guide to determine if your proposed alteration is a Level II or III change. Level II requires local approval only and does not need SUNY or NYSED approvals.
  • * Action requested
Deactivation, Discontinuance and offering a program online is considered a Level III change and will require SUNY and NYSED approval.
  • Discontinue vs. Deactivation (Level III Curricular Change)

Discontinue:  A campus no long offers the program nor awards a credential for completion of the program.  The program is removed from the State Education Department’s Inventory of Registered Programs.

Deactivation:  A campus makes a decision not to admit any more students to a program but wishes to maintain the program’s registration.  This may be done to reassess the need for the program or restructure a program.  This action is internal to SUNY and limited in duration to no more than three years.  The institution must contact SUNY within three years of the deactivation date to officially discontinue or reactivate this program.

  • STEP 3.  IMPORT PROGRAM INFORMATION

DO NOT type in your program or core name.  You must Import current program/core information.  After importing, in the prospective curriculum field below, all current courses will be displayed, as well as all the cores (and courses within each core).

  • Program Name (Must Import Program)*
    Physical Education [PHE] M.S.T.
    Physical Education [PHE] M.S.T.
IMPORTANT: Please import current program information from the college catalog by: 1. clicking on the blue downward arrow icon (located above in this pane); 2. click on your desired catalog; 3. filter the program by name. After returning to Curriculog, you are able to continue to make edits/changes to the pre-populated fields, as well as complete all required fields (denoted by an asterisk) in this proposal.  When making edits to each field, you may be prompted to click the Save button.

  • * Provide summary of proposed changes AND justification for each change.

Graduate-level student teaching experience is being split into 2 different courses; PED 658 and PED 659 as requested by SRRS to eliminate the need for all of the behind-the-scenes work and lifting of registration restrictions. Per SRRS this is how other departments have their student teaching experiences for the purpose of registration. The course(s) content remains unchanged.

There is also a request to change the title of PED 658 - this change will need to be reflected in the program section also.
